Should the employee have any situation arise where they are unsure if they should sign the form one way or the other, they must discuss their concern with the Foreman/Supervisor in charge to determine which side of the form is appropriate to sign. The Foreman/Supervisor will determine that, if any, investigation must take place at that time. Refusal to sign the document one way or the other is not acceptable.
Accidents involve a specific event. Occupational injuries are not caused by a specific accident and generally occur over long periods of time or exposure. In either situation, the employee must report their injury to the Foreman/Supervisor in charge immediately upon becoming aware of it.
