  • Immediate Accident/Injury Reporting Policy

    Woodmark, LLC requires immediate reporting of all accidents involving property damage or personal injury. Should an employee be involved in an accident while at work, the injured employee must immediately report the accident to the Foreman or Supervisor in charge. Employees shall report any incidents as they occur. DO NOT WAIT until the end of the work shift to report the incident. If there is a situation where an employee does not become aware of their injury right away, the employee must report the injury immediately upon becoming aware of it, no later than the end of that business day.

    Woodmark, LLC requires its field employees sign an employee accident sign-out form entitled “Employee Daily Occupational Safety Log For Injuries” at the end of each work shift. This form is used to verify that no workplace accidents or injuries took place on that day. This practice promotes the safest possible work environment for our employees and permits us to promptly and effectively response to any workplace accidents or injuries.

    After every work shift, before leaving the jobsite, each employee must sign the “Employee Daily Occupational Safety Log for Injuries”. The employee must sign either the left side, which indicates the employee did not have an accident or sustain an injury while at work or became aware that day he/she sustained a work related injury. If an employee was involved in an accident or sustains an injury on the job, they must discuss it with the Foreman/Supervisor in charge, complete the accident investigation process, and follow the post-accident/injury drug and alcohol policy testing before leaving work.

    To the extent an employee refuses to sign the “Employee Daily Occupational Saftey Log for Injuries” or otherwise participates in the Company’s protocol for investigations accidents and injuries, the employee with be subject to discipline up to and including termination. Should the employee have any situation arise where they are unsure if they should sign the form one way or the other, they must discuss their concern with the Foreman/Supervisor in charge to determine which side of the form is appropriate to sign. The Foreman/Supervisor will determine that, if any, investigation must take place at that time. Refusal to sign the document one way or the other is not acceptable.

    Accidents involve a specific event. Occupational injuries are not caused by a specific accident and generally occur over long periods of time or exposure. In either situation, the employee must report their injury to the Foreman/Supervisor in charge immediately upon becoming aware of it.

    Significant goals of this policy include, but are not limited to, immediate identification of accidents and injuries to provide prompt medical treatment to the individuals, conduct an immediate accident investigation to determine the “root causes” for the accident or injury, take corrective action to prevent additional accidents or injuries on the job, assist in the enforcement of the Company’s drug and alcohol free workplace policy for the safety of all employees, and to further protect the Company by documenting the scene of the accident, and preserve any necessary evidence to protect the company and preserve its rights.

  • Woodmark Safety Disciplinary Policy

    All associates of Woodmark, LLC are responsible for maintaining a safe work environment. The safety Disciplinary Policy was developed to ensure that employees are held accountable for any unsafe work habits or conditions. To maintain a safe working environment, employees who fail to comply with established safety rules and procedures, and/or have repeated violations, will receive disciplinary action up to and including termination of employment

    In addition, the following items are crucial to maintaining accountability in safe work environments

    1. 1. All accidents must be reported to your supervisor at the time they occur. This includes reporting any personal injury to yourself as well as injuries to others that you may have witnessed. Reporting an accident late (more than 24 hours after it occurred) is considered a serious violation of safety rules and is subject to disciplinary action.

    2. 2. Any unsafe condition, hazard, or safety rule violation noted in the work environment must be reported to the supervisor to correct the problem. The work environment includes Woodmark, LLC property and extends to the jobsites. Any unsafe condition will be remedied prior to proceeding with the job.

    3. 3. Employees are required to wear or use safety equipment specified by your supervisor when operating equipment or working in a specified area. If you are not wearing or using your safety equipment and are involved in a work related accident, you will be subject to disciplinary action

    4. 4. Employees who demonstrate a persistent disregard to established work practices, or in inability to consistently work safely and accident free, will be subject to disciplinary action, up to and including termination.

  • Rescue Procedures for Employees Who Have Experienced A Fall Arrest

    In the unlikely event that a fall arrest occurs, the first goal is to get the employee back to the safety of the ground. If it can be accomplished by following current safety procedures, on-site personnel should attempt to rescue the employee with the use of a ladder, scaffold or articulating man lift if feasible.

    Immediately upon an accident occurring, it is a Woodmark requirement that the following individuals be notified.

    1. 1. 911 RESCUE PERSONNEL IF REQUIRED FOR ASSISTING IN THE RESCUE
    2. 2. JOBSITE FOREMAN
    3. 3. SUPERINTENDENT (AUGIE 630-364-3731)
    4. 4. WOODMARK OFFICE (630-444-0146)

    Once an individual has experienced a fall and they are still in their harness and attached to the retractable, assist the individual in getting his weight off the harness, and get him supported by his feet with the use of a ladder properly staged beneath him so that he can stand on it. If a ladder is not feasible, an attempt should be made to construct a scaffold underneath the individual so that he can stand. If the site has articulating man life, which can assist in the rescue, ensure that the machine manufacture safety guidelines are followed while using the machine in the rescue attempt.

    No individual shall put themselves in an unsafe condition as they are attempting to rescue the person that has just experienced a fall. If a rescue attempt will put either the individual attempting the rescue or the individual that has just fallen into greater danger, you should do your best to comfort the individual that has just fallen but wait for trained emergency personnel to show up at the site to rescue the individual. In this event, please make sure that people are stationed outside of the structure in which the accident has occurred to guide the emergency personnel in the quickest route to the distressed individual.

    At the beginning of any work activity where fall protection is needed, rescue plans must be identified and discussed with all employees in case of a fall. The foreman will develop the rescue plan. Please ensure that the address of the structure is displayed at the site for emergency purposes.

    All employees involved in a fall arrest or fall will be sent for a medical evaluation to determine the extent of the injuries, if any.

  • Post-Accident Drug Testing

    Effective immediately, Woodmark has instituted a post-accident drug testing policy. Immediately after an accident occurs that requires an employee to receive medical attention we will now require the medical office to perform a drug test. By singing below, you agree that you have been aware of this policy

  • Drug/Alcohol Testing Policy

    Any of the situations listed below ill result in you (the employee) being required to submit to a drug/alcohol test while employed with Woodmark, LLC.

    1. 1. Any injury occurring on the job site that requires medical attention away from the job site, lost time from work or accidents reported late.
    2. 2. Motor vehicle accidents with company equipment
    3. 3. Accidents involving cranes, forklifts, aerial lifts, etc.
    4. 4. Accident with company vehicles
    5. 5. Any employee involved in, or causing an accident, injured or not, may be tested at the discretion of the company management.
    6. 6. Any employee showing obvious signs of being under the influence of alcohol or drugs.
    Consequences
    1. 1. Refusing to be tested will terminate your employment with Woodmark, LLC
    2. 2. If you test positive for drugs or alcohol, you will receive disciplinary action, up to and including termination.
  • Reflective Vest Policy

    In an effort to further protect our employee, we have created this policy which requires employees of Woodmark, LLC to wear reflective vests during certain activities. ALL employees that are working around a field forklift that is in operation or any employee that is engaged in the activity of landing or bracing trusses while the crane is present MUST wear a reflective vest. By signing below, you agree that you have been made aware of this policy and that you understand it is your responsibility to ensure that you are in compliance. Any employee that is found to not be in compliance will face disciplinary action up to and including possible termination of employment.

  • Direct Deposit Policy

    New employee needs to provide VOID check or Direct Deposit Form from bank to sign up for direct deposit.

    Automatic deposit of your payroll check to your checking or savings account is available through our payroll system. Once you are set up with Paylocity you can download the app and register to view your paystubs.

    If you do not have checking/savings bank account, please let office manger know to make other arrangements. Check will be mailed via USPS.

    No employee may deposit/cash live check before payday (Friday). Failure to comply with this rule will result in disciplinary action.
